Salary Range:$84000 - $89000 annually. Compensation will be commensurate upon experience and qualifications. This position is Exempt from the Fair Labor Standards Act (FLSA) overtime provisions.

Summary

The Nurse-Family Partnership (NFP) Nurse Home Visitor (NHV) is responsible for providing comprehensive nursing services to families eligible for the NFP Program in the clients home or other community setting. Nurse Home Visitors are responsible for maintaining the highest standards in clinical nursing practice and adherence to the NFP model and to policies procedures guidelines and standards of NFP and of the Helen and Arthur E. Johnson Beth-El College of Nursing and Health Sciences. This position is 80% clinical practice as a NHV and 20% clinical nursing instructor (clinical instruction and service activities) which allows for integration of students into the NHV practice role. The workload for the position is 80% clinical practice 10% service and 10% teaching.

Essential Functions

The duties and responsibilities of the position include but are not limited to:

  • Provide home visits to families eligible for the NFP program while adhering to the fidelity of the NFP model of home visitation.

  • Carry a caseload of 20 clients.
  • Serve as a clinical preceptor for students during home visits and care conferences.
  • Support policies procedures guidelines and standards of NFP and the Helen and Arthur E. Johnson Beth-El College of Nursing and Health Sciences.
  • Participate in community coalition and advisory board activities in support of NFP.
  • Promote public awareness of the NFP program.
  • Develop and maintain community relationships to support client referrals.
  • Maintain current CPR certification for adults children and infants.
  • Complete all required NFP education/training.
  • Develop therapeutic relationships utilize concepts of reflection and motivational interviewing with clients and their families in a home visiting environment.
  • Follow the nursing process in implementing the NFP program.
  • Assess physical emotional social and environmental needs of clients and their families as they relate to the NFP domains.
  • Assist clients enrolled in the FNP program to establish goals and outcomes.
  • Provide education support and referral resources in assisting clients and their families in attaining their targeted goals.
  • Consult and collaborate with other professionals involved in providing services to clients and their families.
  • Formulate nursing diagnosis based on nursing assessment and client goals and evaluate client progress toward stated goals and NFP outcomes.
  • Plan home visits in accordance with client goals and NFP outcomes.
  • Actively engage in professional development to meet all NFP nurse home visitor competency requirements travel may be required.
  • Utilize reflective practice in supervisory sessions.
  • Attend and participate in case conferences and team meetings.
  • Provide information to support staff for timely and accurate data info for the NFP performance management system.
  • Participate in quality improvement efforts.
  • Perform related duties as assigned or required.
  • Understand support and coach others in the NFP vision mission and model.
  • Maintain clear effective open honest communication with both internal and external stakeholders and clients.
  • Create maintain and support a safe environment for open discussion.
  • Seek and respond appropriately to feedback

  • Provide clinical instruction for students and evaluate their performance as directed.
  • Coordinate student schedules and experiences with the lead faculty of clinical courses.
  • Attend departmental and college meetings.
  • Travel to meetings and client visits in El Paso/Teller counties in personal vehicle.

This position is dependent on the availability of external grant funding. Funding is currently expected to last 1 year; however the NFP program at UCCS is in its 11th year of funding.

Tentative Search Timeline

  • Priority will be given to applications submitted by:November 30 2025.
  • Potential interview dates:December 8 - 19 2025.
  • Potential start date: January 2026.

  • Registered Nurse with an active unencumbered Colorado nursing license (or ability to obtain by date of hire) is required.
  • A Bachelors Degree in Nursing from a nationally accredited program is required.
  • Master of Science degree in nursing or health related field from a nationally accredited program is required.
  • Current healthcare provider BLS certification is required.
  • Work experience in maternal/child with diverse populations communities and/or organizations is required.

  • Valid drivers license and insured automobile is required.
  • NFP or other evidenced-based home visiting model experience is preferred.
