JOB TITLE: Transition Coordinator
EMPLOYMENT TYPE: Full-Time 40 hours/week

ABOUT THE PROGRAM: New Life Center
CHRs New Life Center is a Residential Substance Abuse Treatment program designed to accommodate women with substance abuse and co-occurring mental health disorders who have infants toddlers under the age of 4yo and/or expectant mothers. Children reside in the facility with their Mothers in order to promote a healthy parental bond with the focus of developing effective parenting skills. Estimated length of stay is 4-6 months with the goal of preparing for a healthy and safe substance free lifestyle. DCF involved Mothers welcomed. Mothers and infants receive medical care at Day Kimball Hospital.
ABOUT THE POSITION: Transition Coordinator
Duties & Responsibilities
- Work with referral sources to gather needed information for incoming referrals.
- Collaborate with clinical program director regarding appropriateness of potential referrals.
- Facilitates referrals and networks to referral sources to ensure program remains at a minimum of 90% program capacity.
- Engage people with substance use disorders in treatment.
- Orient new clients to treatment services and facility
- Monitor substance use by developing relationships with people performing toxicology screens accessing laboratory results gathering information from other treatment providers.
- Assist in networking individuals into community-based wrap around services to build on and maintain recovery.
- Ensure that people have access to recovery resources including transportation benefits housing employment social supports to enhance recovery.
- Assist in authorizations as needed.
- Coordinate care with external and internal providers including medical and dental care.
- Perform all documentation according to agency standards.
- Other duties as assigned.
QUALIFICATIONS:
Education: Licensed Licensed eligible masters degree in human services or related field associate or bachelors Degree in human services field or related field plus 2 years of full-time experience in SUD services or certified peer (from CCAR Advocacy Unlimited or HHC).
Experience: Excellent communication skills. Excellent computer skills.
Licensure/Certification/Registration: Valid drivers license required.
