About the role
As a Senior Mental Health Support Worker youll play a key role in supporting individuals with complex mental health needs in a supported living environment. Youll work directly with individuals in both long- and short-term supported living services coordinating care plans and ensuring the highest standards of person-centred support and achieve personal goals.
Key responsibilities include:
Providing emotional and practical support tailored to individual needs
Supporting with daily living skills medication and appointments
Promoting independence dignity and choice
Acting as a key worker and providing person-centred support
Supporting individuals with daily living medication tenancy management and wellbeing
Facilitating care reviews and maintaining safe clean environments
Working flexibly across services including evenings weekends and sleep-ins
