Job Description Summary:


Manages the care coordination team and social work activities including patient care case review care plan creation and evaluation performance feedback and professional development. Practices professional social work and promotes advocacy in accordance with the NASW Code of Ethics for Social Work.


Job Description:


Essential Functions:

  • Oversees care coordination team in documentation patient care staffing and clinical competencies. Serves as a clinical expert for policies procedures documentation and skills. Assists in the training and development of new and existing staff members.
  • Assists in supervising clinical operations. Explains why clinical and financial management are important to the organization.
  • Engages in the workplace community by staying aware of changes participating in data collection or auditing understanding Quality Improvement and representing staff at relevant meetings and then reporting back important information and updates.
  • Maintains safety and quality as priorities providing auditing and education for safety and quality initiatives. Ensures regulatory compliance and emergency response preparedness.

Education Requirement:

MSW required.

Licensure Requirement:

  • LSW required.
  • LISW preferred.
  • Valid Ohio drivers license and proof of auto insurance as required by hospital policy and position-specific requirements. Must pass motor vehicle background inspection insurance eligibility driving qualifications and training set forth by Nationwide Childrens Hospital and maintain qualification of insurance guidelines.

Skills:

  • Ability to communicate (verbally and written) clearly convey information to team members healthcare providers and families with professionalism and tact.
  • Ability to coach team members through change provide feedback (growth opportunities and recognition) delegate tasks effectively and make informed decisions.
  • Foster collaboration and positive working relationships within the team. Ability to relate to diverse age and demographic backgrounds.
  • Ensure adherence to relevant regulations and clinical standards.
  • Ability to analyze complex situations identify potential problems and developed solutions for best outcomes.
  • Computer literate with working knowledge of Microsoft products (i.e. Outlook Word PowerPoint Excel and Teams).

Experience:

  • 3 years experience in pediatric clinical care or case management required.
  • 1 year experience in leadership preferred.
  • Electronic Health Record experience preferred.
  • Working knowledge of Medicaid and other regulatory agency standards preferred.
