As a Registered Nurse Case Manager youll be the bridge between medical care and human comfort guiding patients and families through their journey with empathy skill and dignity. Youll collaborate closely with a team that truly values your voice your expertise and your heart.

What Youll Bring
Graduate of an accredited nursing program (Certificate Diploma Associate or Bachelors)
Current RN license in the State of Michigan
At least 1 year of clinical experience (hospice or home health preferred)
Compassion flexibility and a heart for meaningful work
Valid drivers license insurance and reliable transportation (50% travel required)
Ability to lift move and support up to 50100 lbs. as needed
