Job Summary
LeeKO Home Health LLC provides exceptional home health services helping patients regain independence and improve quality of life. We are seeking a skilled Occupational Therapist (OT) and Occupational Therapist Assistant (COTA) to join our team.
Responsibilities OT:
- Deliver in-home occupational therapy per physician orders.
- Develop treatment plans to improve daily living skills.
- Educate patients and caregivers on exercises and mobility.
- Document visits in Synergy EMR.
- Collaborate with interdisciplinary team members.
Responsibilities COTA: (COTA)
Support OT in patient care plans. Document progress accurately. Strong documentation skills. Document visits in Synergy EMR.
Requirements
Qualifications OT:
- Florida OT license.
- 1 year clinical experience (home health preferred).
- Strong communication documentation and organizational skills.
- Reliable transportation.
Qualifications COTA:
Florida COTA license.
