Exceptional Skills and Qualifications
- Must be a graduate of Accreditation Commission for Education in Nursing or Commission on Collegiate Nursing Education accredited school of nursing.
- Licensed as a Registered Nurse by the Indiana Professional Licensing Agency required.
- 3 years: nursing experience required preferably in Population Health management; home health ambulatory care community public health case management and coordinating care across multiple settings and with multiple providers.
- Experience with electronic health records EPIC experience preferred.
- Knowledge and experience teaching self-management preferred.
- Experience working with a Population Health Team &/or Accountable Care Organization preferred.
