Job Summary
Part-time - 4 days/week (Hybrid)
Play a key role in improving diabetes outcomes for Detainees through education support and evidence-based care. Work collaboratively with multidisciplinary teams to empower Detainees in managing their condition effectively.
Join us in bringing a new approach to medical and wellbeing care for detainees by:
1. Clinical Care and Management
Conduct assessments develop care plans and provide direct diabetes management including medication insulin and glucose monitoring in collaboration with healthcare teams.
2. Education and Health Promotion
Deliver education sessions and develop resources to enhance diabetes awareness prevention and self-management across the detention network.
3. Policy and Collaboration
Develop and review diabetes-related policies and provide clinical leadership to ensure consistent high-quality evidence-based care.
Lets talk about you!
- Post graduate qualifications in Diabetes Management and Education
- Minimum of a bachelors degree in nursing Nutrition or a related healthcare field.
- Minimum 5 years of experience in diabetes education and management particularly in settings serving diverse or underserved populations
