WhatYoullDo
- Deliver personalized high-quality care to older adults in homes and senior living settings.
- Conduct comprehensive assessments and develop individualized care plans.
- Manage chronic conditions with skill and empathy.
- Collaborate across clinical disciplines (primary psych palliative neuro) for whole-person care.
- Buildtrustedrelationships with residents caregivers and community partners.
- Maintain thorough documentation and consistent communication.
WhatYoullBring
- Active unencumbered NP or PA license in the state you will be practicing
- Minimum of 2 years of clinical experience (geriatrics or primary care preferred)
- Compassionate patient-centered approach and strong clinical judgment
- Valid drivers license and reliable transportation
