A Care Navigator is a key member of the FullWell Care Management team that will support the RN care manager(s) and help outreach to patients following ED visits in-patient hospitalization and stable disease management patients. This role will coordinate with the patients primary care provider for medication refills post- hospitalization appointments and any RN delegated duties etc. This role requires a strong telephonic communicator who can speak to patients to encourage/schedule preventive screenings coordinate with primary care practices to schedule visits assist patients with their care needs and partner with their assigned RN care manager. We are looking for someone with initiative who works well with our Care Management team to ensure excellent quality patient care with great outcomes.

MA or LPN (Required)
Sedentary work - prolonged periods of sitting and exert up to 10 lbs. force occasionally
Our dynamic sports medicine team is seeking a dedicatedRemote Triage Coordinator (LPN or MA) to join this vital role you will be the first point of contact for patients seeking specialized care expertly guiding them to the appropriate sports medicine provider.
- Answer incoming calls to our triage line empathetically listening to patient needs and concerns.
- Utilize your clinical knowledge and critical thinking to assess patient symptoms and determine the most suitable sports medicine team member for their care (e.g. physician physical therapist athletic trainer).
- Directly schedule patient appointments into EPIC ensuring a seamless and efficient process
