Position Summary: The Licensed Master Social Worker (LMSW) provides comprehensive psychosocial assessment crisis intervention counseling and discharge planning services to patients and their families within the Adult Comprehensive Psychiatric Emergency Program (CPEP). The LMSW collaborates with an interdisciplinary team to ensure high-quality patient-centered care and facilitates appropriate referrals and follow-up services to support recovery and stabilization.
Key Responsibilities
Conduct psychosocial assessments and develop individualized care and discharge plans for patients presenting in the psychiatric emergency setting.
Provide crisis intervention and short-term counseling to patients and families in distress.
Coordinate care with physicians nurses psychiatrists and other healthcare professionals to ensure appropriate patient disposition and continuity of care.
Facilitate referrals to inpatient outpatient and community mental health services ensuring smooth transitions across levels of care.
Document all assessments interventions and referrals accurately in the electronic medical record (EMR) in a timely manner.
Participate in multidisciplinary team meetings treatment planning and case conferences.
Advocate for patient rights and access to appropriate mental health and social services.
Maintain knowledge of community resources psychiatric facilities and social service agencies to provide comprehensive discharge planning support.
Comply with all hospital policies ethical standards and state/federal regulations governing social work practice.
Engage in continuing education and professional development to maintain licensure and stay updated on best practices in psychiatric social work.
Required Skills & Experience
Minimum 1 year of Licensed Master Social Worker (LMSW) experience required.
Prior experience working in a hospital setting and Adult CPEP (Psychiatric Emergency Program) strongly preferred.
Demonstrated ability to perform under pressure and effectively manage psychiatric crises.
Strong assessment counseling and documentation skills.
Excellent interpersonal and communication skills with the ability to work collaboratively as part of a multidisciplinary team.
Working knowledge of community mental health systems and crisis intervention techniques.
Education: Masters Degree in Social Work (MSW) from an accredited institution Required
Certifications & Licensure
Valid Licensed Master of Social Work (LMSW) license issued by the New York State Education Department (NYSED) Required
