Job Summary

Location: Montour Falls NY
Unit: Long-Term Care / Skilled Nursing
Duration: 13 Weeks
Shift Details:
Shift Type: Evening Shift
Schedule: 3:00 PM 11:00 PM
Shift Structure: 58-hour shifts
Hours per Day: 8 hours
Hours per Week: 37.5 hours
Schedule Notes: Every other weekend & every other holiday required
Compensation:
Weekly Gross: $1730/week
Job Description
We are seeking an experienced Licensed Practical Nurse (LPN/LVN) to join a Skilled Nursing/Long-Term Care unit. The ideal candidate will deliver compassionate and high-quality nursing care while supporting residents with daily needs and clinical requirements in a fast-paced long-term care setting.
Key Responsibilities:
Provide direct nursing care to residents in accordance with care plans and regulatory guidelines
Administer medications and treatments as prescribed
Assist residents with activities of daily living (ADLs) when needed
Monitor and document resident health status reporting any changes promptly
Collaborate with RNs CNAs and interdisciplinary staff to ensure continuity of care
Maintain accurate and timely documentation within the EMR system
Ensure compliance with infection control and facility safety procedures
Participate in emergency response procedures as required
Provide supportive communication and compassionate interaction with residents and families
Requirements:
Minimum 2 years of LPN/LVN experience (Required)
Active LPN/LVN License (State Requirement)
PointClickCare (PCC) experience required
Strong clinical judgment and communication skills
Ability to work evenings weekends and holidays as scheduled
