Location: Logan Health Medical Center Kalispell Montana

Shift: Day Shift 8 Hours FullTime 40 Hours

The Specialty RN Navigator serves as a clinical guide and patient advocate within the Imaging department with a focused emphasis on breast health and lung nodule tracking. This role ensures timely follow-up seamless care coordination and adherence to screening and diagnostic guidelines for mammography breast imaging and CT lung nodules. You will support patient flow clinical quality and coordinated care across Mammography Ultrasound Breast MRI and connected Bass Breast Center services.

What Youll Do

Breast Imaging Navigation (Mammography Ultrasound Breast MRI)

  • Serve as the primary point of contact for patients with abnormal imaging findings.

  • Coordinate follow-up imaging biopsies localizations and consultations with breast specialists.

  • Educate patients on breast health screening guidelines contrast-enhanced mammography (CEM) MRI procedures and diagnostic pathways.

  • Collaborate with radiologists surgeons oncologists technologists and primary care teams to ensure smooth handoffs and timely care.

  • Monitor daily mammography ultrasound and breast MRI schedules to verify correct exams appropriate orders and proper sequencing for patients with multiple appointments.

  • Assist with procedure preparation including IV starts for contrast exams biopsy and localization tray setup and lymphatic injection procedures.

  • Support radiologists and technologists during procedures (biopsies CEM MRI guidance lymph injections) and serve as a chaperone when needed.

  • Complete post-biopsy check-in calls relay results per radiologist guidance and enter outcomes into PENRAD.

  • Maintain awareness of Tumor Board patients gather prior imaging and pathology when needed and coordinate follow-up prior to case review.

  • Serve as a reassuring and knowledgeable resource for patients helping support anxiety management and service recovery when needed.

Lung Nodule Tracking & Compliance

  • Maintain and monitor a registry for patients with incidental or screening-detected lung nodules.

  • Ensure timely follow-up imaging and adherence to Fleischner Society or Lung-RADS recommendations.

  • Communicate recommended surveillance timelines to ordering providers.

  • Facilitate referrals to pulmonology or thoracic surgery when appropriate.

  • Support quality improvement initiatives related to lung cancer screening and ongoing follow-up compliance.

Program Support & Quality Initiatives (Desirable)

  • Serve as a reliable point of contact for breast health outreach programs.

  • Monitor follow-up patients to support completion of recalls recommended imaging and high-risk surveillance.

  • Assist with accreditation readiness data tracking and quality monitoring.

  • Create and maintain patient preparation sheets procedural checklists and communication tools to support efficient workflows.

General Responsibilities

  • Provide compassionate patient-focused education and support throughout the care journey.

  • Document accurately in the EMR and tracking systems.

  • Participate in multidisciplinary meetings case reviews and departmental initiatives.

  • Help support organizational goals related to early detection care coordination and exceptional patient experience.

Required Qualifications

  • RN with current Montana licensure

  • 3 years of clinical experience

  • Strong communication organizational and EMR documentation skills

  • Ability to provide compassionate support and patient advocacy in a high-touch role

Preferred Qualifications

  • Prior experience in breast imaging oncology or womens health navigation

  • Familiarity with:

    • Breast imaging workflows and procedures

    • MRI and contrast-enhanced imaging

    • Lung cancer screening protocols

  • Previous experience in patient navigation or care coordination

  • Ability to support IV starts and procedural preparation

Qualifications:

  • Current Montana RN License or a multi-state compact license with authorization to practice nursing within the state of Montana; required.

  • Minimum of one (1) of the following required:

    • Bachelors degree in a related field and four (4) years experience.

    • Minimum of six (6) years experience in a related area.

  • Minimum of four (4) years recent specialty experience in a minimum of one (1) of the identified specialty areas required:

    • Cardiology

    • Clinical

    • Endocrinology

    • Gastro

    • Oncology

    • Orthopedics

    • Pediatrics

    • Pulmonology

    • Surgery

  • Knowledge of professional practice standards and regulatory requirements as related to specialty care area required.

  • Knowledge and experience with electronic health records required.

  • Must function with a high degree of autonomy strong communication and interpersonal skills. Possess the ability to develop and implement community education outreach programs/workshops and seminars as defined for assigned specialty area required.

  • Possess ability to comprehensively assess patients psychosocial status and demonstrate an understanding of the physiological disease practice as defined for assigned specialty area(s) required.

  • Demonstrated ability to work collaboratively with Primary Care teams and/or multidisciplinary care teams required.

  • Promotes excellence through setting high standards and providing high quality outcomes.

  • Ability to act with integrity kindness and exhibit empathy.

Job Specific Duties:

  • Evaluates the needs and coordinates the care of the patient through the continuum of care by ensuring patient needs are met.

  • Works closely with members of the multidisciplinary care team and their recommendations. Exhibits professionalism and accuracy in interfacing with providers clinic staff patients and family members involved in the plan of care.

  • Documents interaction per professional standards related to pre/post care and/or survivorship plan as applicable. Assesses the physical psychological social and spiritual needs of the patient and family.

  • Identifies and assesses the learning needs of the population under care and/treatment. Builds therapeutic and trusting professional relationships with patients families and caregivers. Utilizes motivational interviewing techniques to assist patients in meeting goals and managing chronic disease as defined for assigned specialty area(s).

  • Assists with the organization of the specialty interdisciplinary team meetings and facilitates the teams recommendations in a professional and timely manner.

  • Assists with specialty tests per standard and procedure as defined for assigned specialty area(s).

  • Works collaboratively with all stakeholders to identify patient needs and assists in care and services both medical and social throughout the continuum.

  • Collaborates with the navigation team and leadership to optimize processes and metrics to; reduce avoidable ED visits inpatient admissions hospital readmissions cost of care gaps in care increase patient satisfaction clinical outcomes and provider satisfaction as defined for assigned specialty care area(s)

  • Assesses education needs and seeks to update knowledge base to provide quality and current patient specific specialty service line coordination.

  • Facilitates the use of telehealth and/or self-monitoring technology to enhance the plan of care and patient engagement with the assigned specialty care team as needed.

  • Actively participates in activities that add value to organizational objectives and workflows such as process improvement projects and/or community-related initiatives to include community health fairs workshops etc.
