Job Summary
Hospice After Hours Nurse
Salary: $80-89k/yr BOE
Schedule: Full-time Monday- Thursday 4:00 PM-8:00 AM
Location: Cincinnati Ohio (surrounding area)
Requirements of the After Hours Nurse role:
Licensed RN in the state of practice
BLS Certified
A minimum of 1 year of Clinical/acute care experience
Valid drivers License and Auto insurance Coverage
Reliable transportation: Willing to travel
On-call/ triage experience preferred
About the After Hours Nurse role:
Provide on-call services after hours and complete skilled visits if needed.
Coordinate hospice care by working with other team members as well as with the patients family members to ensure needs are met.
Triage calls coordinates services and provides frequent updates on plan of care and any changes in condition.
Assist reinforcement of patient and family education regarding end-of-life care.
Participate in IDG and case conferences when needed
Assume responsibility for personal and vocational growth and development through active participation in nursing organizations and participating in in-service programs and other on-the-job learning.
Keep the RNCM informed of changes noted during the process.
Participate in self-development by practicing further education workshops and conferences active in professional and related organizational meetings and individual research and reading.
Respond to on-call or medical emergencies (nights and weekends)
