Psychiatric Mental Health Nurse Practitioner (PMHNP) Queens NY

Requirements
Conduct comprehensive psychiatric evaluations and regular follow-up visits for residents in skilled nursing and post-acute care environments.
Develop and adjust personalized treatment plans that meet each patients unique mental health needs.
Prescribe monitor and manage psychotropic medications ensuring safety and therapeutic effectiveness.
Work collaboratively with an interdisciplinary care teamincluding physicians nurses psychologists and social workersto coordinate holistic care.
Provide therapeutic guidance counseling and education for patients and families to support treatment adherence and understanding.
Maintain accurate and compliant documentation in the electronic health record (EHR) system.
Participate in team meetings case conferences and quality improvement initiatives to promote best clinical practices.
Qualifications
Active New York State Nurse Practitioner license with Psychiatric-Mental Health certification (PMHNP).
In-depth knowledge of psychopharmacology and evidence-based treatment for geriatric populations.
Excellent assessment communication and collaboration skills.
Comfortable working both independently and as part of a team-based care model.
Proficiency in EHR systems and general digital literacy.
