As a vital member of our care coordination team you will specialize in working directly with patients and patient groups collaborating to create individualized integrated healthcare plans and ensuring seamless follow-up. Youll be the crucial link between patients healthcare providers and community resources making a tangible impact on the well-being of our community. This role demands a proactive approach requiring you to navigate various healthcare settings and community agencies occasionally working non-traditional hours to accommodate patient needs.
What Youll Do (The Core of Your Impact):
- Patient-Centered Advocate:Work directly with patients to develop and implement personalized integrated health plans ensuring compliance with health management goals through ongoing support and follow-up.
- Community Connector:Conduct essential health education and screening outreach efforts within the community driven by referrals from case management and healthcare providers.
- Barrier Reducer:Actively identify and mitigate client barriers to accessing necessary healthcare services and vital community resources.
- Collaboration Catalyst:Facilitate effective communication and coordination between a diverse range of community resources/services and healthcare providers.
- Resource Expert:Maintain a comprehensive and up-to-date knowledge base of all available community resources services and programs to effectively serve our patient population.
- Vulnerable Population Champion:Create crucial connections between vulnerable individuals and the community services and resources they need most.
- Program Developer:Assist in establishing a unified referral system and contribute to the ongoing evaluation and improvement of the community health worker program.
- Team Player:Work closely and collaboratively with Providers and care coordination teams to ensure holistic patient care.
- Cultural Competence:Demonstrate a high level of cultural competence fostering an inclusive and diverse environment for all patients and colleagues.
- Strategic Initiatives:Lead committees work groups and/or projects focused on implementing impactful educational activities and program improvements.
- Technology Integrator:Facilitate the effective use of technology including virtual methods to support all aspects of program delivery.
- Data-Driven Decisions:Develop processes for the systematic and purposeful collection and dissemination of data and information to inform program development and resource allocation.
- Strategic Partnerships:Allocate resources based on internal and external environmental scanning cultivate a strategic partnership network aligning with the systems workforce plan and build and maintain strong internal partnerships (e.g. HR Quality Improvement Learning & Development).
- Learning Environment Cultivator:Ensure a healthy multi-disciplinary learning environment across the system.
Job Requirements
What Youll Bring (Qualifications):
Required Education and Experience:
- High school diploma or GED.
Required Licensure and Certifications:
- Community Health Worker Certification (to be obtained within the first three (3) months of hire).
- Current AHA BLS Certification.
Required Minimum Knowledge Skills and Abilities:
- Exceptional Communication:Ability to write clear reports business correspondence and procedure manuals. Ability to effectively present information and respond to common inquiries or complaints from groups of managers clients customers and the general public.
- Program Development Expertise:Demonstrated experience in program development implementation and evaluation.
- High Emotional Intelligence:Possesses strong self-control self-awareness self-management and social awareness.
- Leadership & Mentorship:Ability to identify talent coach and develop others.
- Dynamic Presence:Energetic enthusiastic and passionate serving as a positive role model.
- Mathematical Proficiency:Ability to perform basic arithmetic operations (add subtract multiply divide) with whole numbers fractions and decimals. Ability to compare rates ratios and percentages and interpret bar graphs.
- Problem-Solving & Reasoning:Ability to solve practical problems and adapt to varied situations interpreting various instructions in written oral diagram or schedule form.
- Computer Proficiency:Must be proficient with the Microsoft Office Suite Google applications internet navigation database management spreadsheets Electronic Health Records (EHR) and graphic presentation packages.
Preferred Qualifications:
- Previous experience in a similar role or related healthcare/community outreach field.
