Job Summary:
The Registered Nurse will provide skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. Responsibilities include patient assessments medication administration treatment planning and collaboration with an interdisciplinary team. Strong communication and collaboration with patients providers and team members are essential.

Responsibilities:
- Provide skilled nursing care.
- Perform patient assessments.
- Administer medications.
- Develop and implement treatment plans.
- Collaborate with the interdisciplinary team.
- Provide case management.
- Provide hospice care.
- Float between like units potentially including Pediatrics.
Required Skills & Certifications:
- Active Registered Nurse license in the state of working.
- ACLS BCLS NIH and PALS certifications.
- Minimum of 2 years of experience.
- Clinical Assessment Medication Administration and Treatment Planning skills.
- Interdisciplinary Team Collaboration skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Bachelors degree in Nursing or related field.
- Drivers license car insurance and automobile for field work.
Preferred Skills & Certifications:
- Not specified.
Special Considerations:
- May be required to float to other facilities.
- Weekend shifts every other weekend (6-8 shifts per contract).
Scheduling:
