Job Summary
We are seeking an experienced Registered Nurse Case Manager/Utilization Review/CDI for a 13-week contract position. The RN Case Manager will coordinate and monitor patient care services to ensure quality cost-effective care and appropriate utilization of resources. The role involves collaboration with the healthcare team to manage patient flow support clinical documentation integrity (CDI) and promote optimal outcomes for patients within an acute care setting.
Key Responsibilities
Perform case management and utilization review activities to ensure efficient and appropriate patient care.
Evaluate patient progress medical necessity and readiness for discharge.
Collaborate with physicians nursing staff and other departments to coordinate patient care and discharge planning.
Conduct concurrent and retrospective reviews of patient medical records.
Identify and resolve barriers to discharge and care transitions.
Ensure compliance with hospital policies payer guidelines and regulatory standards.
Utilize EPIC EMR for documentation care coordination and utilization tracking.
Maintain accurate and timely documentation of all case management activities.
Support Clinical Documentation Improvement (CDI) initiatives by working closely with physicians and coders.
Participate in multidisciplinary rounds and case conferences.
Maintain patient confidentiality and adhere to HIPAA regulations.
Perform additional duties as assigned by leadership.
Required Qualifications
Active RN License (New York State).
BLS certification (current and maintainable).
Minimum 12 years of recent acute care case management or utilization review experience (within the past 2 years).
Experience working in a Level 1 or Level 2 trauma hospital preferred.
EPIC EMR experience required.
Excellent interpersonal critical thinking and communication skills.
Ability to work independently and collaboratively within a multidisciplinary team.
Must have legal authorization to work in the U.S. and successfully pass health clearance background and drug screening.
Preferred Qualifications
Prior experience in Clinical Documentation Integrity (CDI) programs.
Strong understanding of payer requirements care coordination and utilization management processes.
Advanced knowledge of medical necessity criteria and discharge planning workflows.
