Position Summary
The Clinical Wellness Coordinator will play a vital role in promoting a holistic trauma-informed approach to participant wellness within the Employment Services program. This newly created position will focus on developing and implementing wellness programming and engagement strategies that complement our existing employment education and financial counseling services.
Reporting to the Program Director and working collaboratively with employment coordinators education specialists and the program clinician the coordinator will ensure participants receive integrated support addressing both their employment and overall well-being.
Qualifications
- Bachelors degree in social work Psychology Human Services or a related field required; masters degree in social work (MSW) strongly preferred.
- Minimum of five (5) years of experience in social services mental health substance abuse or a related field.
- Demonstrated experience in case management and a strong understanding of city agency services and public benefits systems.
- Knowledge of trauma-informed care wellness promotion and service modalities such as Thrive NYCs Connections to Care.
- Proven ability to meet performance metrics and deliver measurable program outcomes.
- Strong interpersonal organizational and communication skills; ability to work independently and collaboratively in a multidisciplinary team.
- Experience in data tracking reporting and using evidence to inform best practices.
- Proficiency in Microsoft Office Suite (Word Excel PowerPoint Outlook) Google Workspace (Docs Sheets Slides Drive) Adobe Acrobat database management systems and virtual meeting platforms (Zoom Microsoft Teams).
Key Responsibilities
- Provide one-on-one case management and service planning to guide participants through job readiness address barriers to employment and coordinate access to public benefits and related services including HRA SNAP HEAP WIC Cash Assistance Medicaid Medicare Food Pantries DHS DMV IDNYC ACS and SSA programs.
- Develop and facilitate community-based initiatives and events to promote mental health wellness and self-sufficiency among Jobs Plus members including hosting a minimum of three workshops per month.
- Build and maintain partnerships with internal programs community-based organizations city agencies hospitals and other service providers to connect participants with appropriate resources and supports.
- Coordinate quarterly professional development training courses for Jobs Plus staff to strengthen awareness of barrier-reduction strategies and holistic service delivery.
- Facilitate the transportation and delivery of resources including food pantry supplies and other materialsfrom Henry Street and partner organizations to the Jobs Plus site.
- Maintain accurate documentation including progress notes client tracking biweekly reports and data entry across relevant databases ensuring timely and compliant submissions.
- Provide crisis intervention suicide prevention support and brief supportive counseling to participants experiencing emotional or traumatic distress referring clients to the program clinician for ongoing care as appropriate.
- Ensure all activities and documentation comply with HIPAA and agency confidentiality policies.
- Attend all required meetings training events and conferences as designated by the funder or supervisor.
- Meet engagement goals by connecting with a targeted number of participants and collaborating with clinicians to complete at least 243 Patient Health Questionnaires (PHQs) quarterly.
- Perform additional duties and special projects assigned by the supervisor.

- Ability to stand for extended periods during workshops and events.
- Capacity to travel to various sites and partner organizations across the five boroughs.
- Must be able to work onsite at the Lower East Side location.
