Data Collection: Unit Information: (insert name) Location: Address: 2425 Samaritan Drive San Jose CA 95124 Floor: Basement Building: Main Hospital General Information Tell us about the unit(s): Number of Beds 4 Cath Lab Rooms Number of Staff 20 RNs Type of staff: Room Mix 2 RN and 2 RT. 8 9 RNs staffed daily o Day Shift 7 3:30 PM Monday - Friday o Night Shift On call only Patient Ratios 2RN 2RT per patient in procedure Type of equipment GE Philips EMR MacLab - Meditech Typical hiring profile Skill Set Most recruited for: Must have: candidates without these skills will not be considered for the role. ACLS BLS NIH Current RN license Preferred or nice to have: candidates with these skills will be considered first. Neuro and Electrophysiology experience List typical procedures performed on unit(s): Cardiac Cath lab heart Cath s Peripheral procedures A-fib/Flutter SVT VT etc. Stroke and complex Neuro intervetions Best personality Fit: Team work REQUIRED CERTIFICATIONS: ACLS BLS Current RN license Floating Area(s): Recover cath lab patients Dress Code: Scrub Color/Attire - Scrubs provided by hospital Unit Guidelines/Policies: Please feel free to attach unit policies and guidelines.
