_{We are seeking a dedicated and compassionate Outpatient Physical Therapist to join our rehabilitation team in Chico CA. This full-time permanent position focuses on providing individualized evidence-based care to patients with a range of diagnoses in an outpatient setting. The Physical Therapist will be responsible for evaluating planning and implementing treatment programs to improve patients mobility function and overall quality of life.}

_{Key Responsibilities:}

_{Perform comprehensive evaluations and develop individualized treatment plans based on physician referrals.}
_{Address impairments related to self-care functional mobility cognition upper extremity function and perception.}
_{Deliver evidence-based interventions to support recovery and promote independence.}
_{Provide caregiver/patient education to ensure safe and effective continuation of care.}
_{Maintain accurate documentation of patient progress and clinical outcomes.}
_{Collaborate with physicians and the interdisciplinary care team to optimize treatment plans.}

_{Education Training & Experience:}

_Required:

_{Graduate of an accredited Physical Therapy program}
_{Current CPR certification}

_Preferred:

_{At least one year of experience in an acute care outpatient rehabilitation extended care or mental health setting}

_Licensure:

_{Current California Physical Therapy License or}
_{California Limited Permit with intent to pass the next available board exam (employment contingent on successful completion)}
