Job Summary:
The Registered Nurse (RN) provides skilled nursing care case management and hospice care to individuals. Responsibilities include patient assessments medication administration treatment planning and interdisciplinary team collaboration. This position requires strong communication and collaboration skills as well as a minimum of two years of experience.

- Providing skilled nursing care
- Case management
- Floating between units as needed
Required Skills & Certifications:
- Clinical Assessment Medication Administration and Treatment Planning skills
- Minimum two years of hospital home health or hospice care experience
- Interdisciplinary Team Collaboration skills
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Active Registered Nurse license in the state of working
- Bachelors degree in Nursing or related field
- Drivers license car insurance and automobile for field work
Preferred Skills & Certifications:
- Not specified
Special Considerations:
- Must have a vehicle for this assignment
- May float to other units based on skill set
- May be required to float to other facilities
- No on-call requirement
Scheduling:
- Weekends required (every other sometimes more)
- Expected ratios: 1:1 or 1:2
- Block schedules not required
