Role : Speech Language Pathologist
Location : Flagstaff AZ
Contract : 6 Months
Day - Mon - Friday 40Hrs a Week
Position Summary:
A Speech Language Pathologist (SLP) will evaluate and treat residents/patients with temporary or long-term speech hearing swallowing and cognitive disabilities in compliance with state and federal guidelines. They will evaluate the residents/patients speech language cognitive-communication voice swallowing and fluency and develop treatment plans. They will treat the residents/patients based on a physicians treatment plan.
Essential Functions:
Screens and evaluates patients/residents and develops appropriate care plan.
Under a physicians direction provides direct patient/resident treatment based on established plan of care.
Plans directs or conducts rehabilitative treatment programs to restore communicative efficiency of individuals with communication and/or swallowing or cognitive problems.
Administers scores and interprets specialized hearing and speech tests.
Instructs residents/patients families and caregivers to monitor speech and provide ways to practice new skills.
Completes required forms and clinical records in accordance with company policy and state and/or federal regulations.
Documents in patient records in an accurate and timely manner.
Devises special tools and treatments for the specific needs of patients.
Provides comprehensive and compassionate therapy services in a sub-acute/skilled nursing facility setting.
Works collaboratively with other rehab therapists and nursing staff in a supportive team-working environment.
Maintains resident/patient privacy pursuant to HIPAA.
Ensures punctuality and regular attendance for assigned shifts.
Requirements:
Bachelor of Science degree in Speech Language Pathology required.
Active license in Speech Language Pathology in the Stat of Arizona.
