Key Responsibilities:
Administer medications and treatments as prescribed following all safety and regulatory protocols.
Provide direct patient care including monitoring vital signs assisting with daily living activities and documenting care accurately.
Observe and report changes in resident condition to the supervising nurse or physician.
Ensure compliance with facility policies state regulations and professional standards of practice.
Assist in creating and implementing individualized care plans.
Communicate effectively with residents families and healthcare team members to promote a supportive and caring environment.
Maintain a clean safe and organized care environment.
Flexibly work evening shifts weekends and multiple consecutive days as required.
Qualifications:
Current and valid LPN license in the state of employment.
Minimum of 12 years of experience in long-term care assisted living or skilled nursing facility preferred.
Knowledge of medication administration patient care procedures and safety protocols.
Strong communication and interpersonal skills.
Compassionate detail-oriented and dependable.
