What you will do:

The Manager Care Management provides supervision and administrative oversight to the Care Management Team and functions as an expert clinical practitioner and skilled manager of clinical systems that impact patient care. The Manager Care Management provides daily management of personnel issues such as performance management and staffing. Under the direction of the Senior Director or above serves as an advisor resource preceptor and leader for members of the Care Management/Care Coordination Team. The Manager supervises the coordination of interdisciplinary care for a population of patients from pre-admission through post discharge and taking into consideration the age and development needs of the patient. Is responsible for the interdisciplinary coordination of patient care as established by critical paths managed care contracts and individualized interdisciplinary plans of care.

you will bring:

  • Masters degree from an accredited school of Nursing or Social Work OR Bachelors degree from an accredited school of Nursing with 4 years experience as a LEAD Case Manager required.
  • Five years full-time nursing including minimum two years experience in specialty area if hired as a Nurse or Five years (of which 2 years are mental health/health related) at the LCSW-C level if hired as a Social Worker. Additional care management/ disease management or managed care experience preferred.
  • Current registered nurse licensure in the State of Maryland or licensed by the Maryland Board of Examiners at the LCSW-C level.

Salary Range: Minimum 51.22/hour - Maximum 79.39/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount salary discussions will occur during candidate screening calls before any subsequent compensation discussion is held between the candidate and any hiring authority.
