Role Summary:
The Director of Case Management will lead daily case management operations driving care coordination discharge planning and utilization review to ensure safe and cost-effective care transitions. The role requires strong collaboration with interdisciplinary leaders to enhance patient outcomes and organizational efficiency.
Key Responsibilities:
Lead and support case management and social work teams to deliver optimal patient outcomes
Oversee utilization management discharge planning and care transition operations
Partner effectively with physicians nursing and administrative leadership
Ensure compliance with regulatory and payer requirements (including CMS)
Create and implement performance improvement initiatives and efficiency strategies
Qualifications:
Active RN license in Florida or Compact state
Minimum of 3 years of leadership experience in case management
Bachelors degree required; Masters degree strongly preferred
