The Social Worker Opportunity:
The Social Worker -Bachelors (BSW) is a strategic member of the treatment team and supports the patient with recommendations and referrals to other social services placements in outside care systems and coordination of care with family members and other providers regarding the needs of the patient.
Compensation and Benefits:
- The pay range for this role is $29.48 - $32.10 based on education and experience.
- This job is full-time and in person.

- As a full-time employee of NCHC you would be eligible for the Wisconsin Retirement System (WRS). The WRS is a hybrid defined benefit plan. It contains elements of both a 401(k) or defined contribution plan and a defined benefit plan. Participating staff contribute 6.95% and NCHC will match this at 100% each pay period!
What Qualifies You:
Minimum Education and Experience:
- Bachelors degree in Social Work or related field earned at an accredited four-year college or university
- Licensed as a social worker by the State of Wisconsin or ability to obtain licensure
- More than one year of social work experience.
- More than one year of experience with discharge planning
Preferred Qualifications:
- More than 3 years social work experience
- Current Wisconsin Substance Abuse Counselor or Substance Abuse Counselor In-Training certification
What a Social Worker does at NCHC:
- Responsible for timely completion of psychosocial assessments Treatment Plan and Treatment Plan Reviews and Discharge Summaries.
- Responsible for appropriate Case Management/Discharge Planning based on patient need and assisting in complex placement issues.
- Makes appropriate and timely referrals to other agencies based on patient need and services required.
- Performs social counseling services for individuals families and groups.
- Makes treatment recommendations to county corporation counsel and Human Service Departments.
- Contributes participates and supports the Interdisciplinary Team (IDT) approach to providing quality care for clients.
- Documents ongoing assessments and updates plan of care to reflect changes in clients condition.
- Counsels with clients families or health care agents of clients regarding the plan of care.
- Serves as a client advocate.
- Attends and participates in assigned treatment staffing and other assigned committees.
- Responds to community requests for information regarding available services.
- Assures that patient care provided in the department is completed in a respectful manner that upholds the dignity and rights for adolescent and adult patients.
- Maintains regular on-site attendance
