Job Summary
Description of the role:
The Client Care Coordinator at Sunlight Senior Care - Omaha is responsible for coordinating care services for our clients to ensure their comfort and well-being. This position involves working closely with clients families and caregivers to create personalized care plans and maintain quality standards.
Responsibilities:
- Assessing clients needs and developing care plans
- Coordinating caregiver schedules and assignments
- Communicating with clients and their families to provide updates and support
- Monitoring the quality of care services provided
- Maintaining accurate client records
- Interviewing On boarding and managing caregivers
- May require providing care to clients in extreme situations
Requirements:
- Previous experience in client care or a related field
- Strong communication and organizational skills
- Compassionate and patient demeanor
- Ability to work independently and collaboratively
- Knowledge of care coordination principles
- Be available after hours for questions calls etc.. (as needed)
- At Least an Associates Degree
- Pass a thorough background check
- Reliable Car
- Valid Drivers license
- >5yrs experience as a HHA CNA PCA
