Job Title: Licensed Practical Nurse (LPN) Long-Term Care (LTC)
Location: Walpole MA
Duration: 13 Weeks (Extendable)
Shift: Day & Evening 5x8-Hour Shifts
Pay Rate: Local: $45/hr Traveler: $52.58/hr

Position Overview:

We are seeking an experienced and compassionate Licensed Practical Nurse (LPN) to join our long-term care team. The LPN will play a key role in delivering quality nursing care to residents supporting their daily health needs and ensuring a safe comfortable and respectful environment.

Key Responsibilities:

Provide direct nursing care to residents under the supervision of an RN or physician.
Administer medications and treatments as prescribed.
Monitor and document residents conditions vital signs and responses to care.
Assist with care planning and communicate changes in resident status promptly.
Maintain accurate and timely medical records in compliance with facility policies.
Support residents with activities of daily living (ADLs) as needed.
Promote a positive and compassionate atmosphere for residents and families.

Qualifications:

Active LPN License in the state of Massachusetts (or compact eligible).
Minimum 1 year of experience in a long-term care or skilled nursing setting preferred.
Current BLS (AHA) certification required.
Strong clinical assessment documentation and communication skills.
Ability to work collaboratively in a multidisciplinary care environment.
