Job Summary

Job Title: Licensed Practical Nurse (LPN) Long-Term Care (LTC)
Location: Woburn MA
Duration: 13 Weeks (Extendable)
Shift: Day 5x8-Hour Shifts
Pay Rate: Local: $45/hr Traveler: $52.58/hr

Position Overview:

We are seeking a dedicated and skilled Licensed Practical Nurse (LPN) to provide high-quality nursing care in a long-term care setting. The LPN will work closely with the healthcare team to ensure residents receive safe compassionate and effective care.

Key Responsibilities:

Provide direct patient care and administer medications as prescribed.
Monitor record and report changes in residents conditions.
Support residents with daily activities (ADLs) and maintain comfort and safety.
Communicate effectively with RNs physicians and families regarding patient needs.
Maintain accurate and timely documentation according to facility standards.
Ensure compliance with all healthcare regulations and facility policies.

Qualifications:

Active LPN License in Montana (or compact state).
Minimum 1 year of experience in a long-term care or skilled nursing facility preferred.
Current BLS (AHA) certification required.
Strong attention to detail time management and communication skills.
Compassionate and patient-centered approach to care.
