The Senior Therapeutic Youth Support will provide supportive services to youth and families across one or more of Advocates CBHI service lines. Service lines include In Home Therapy Therapeutic Mentoring and In Home Behavioral Services. This team member will offer structured strength-based support services to youth for the purpose of addressing daily living social and communication needs. Services may be provided independently or in collaboration with clinicians or other providers. The Senior Therapeutic Youth Support will be a leader on the CBHI Team and will hold additional responsibilities in support of colleagues and the functioning of the team as a whole.

Responsibilities

    • Participate in the development and implementation of Individual Action Plans or Behavior Action Plans.
    • Assist in providing intensive family therapy.
    • Identify community resources and develop natural supports for youth and parent/guardian/caregiver
    • Assist in providing complete care coordination:
      • Attend necessary meetings to include IEP meetings hospital discharge meetings Care Planning Team (CPT) meetings for members enrolled in ICC.
      • Provide collateral contacts for case coordination (phone and face to face)
      • Collaborate with care coordinator (for youth enrolled in ICC services) and/or behavioral service providers for treatment planning
      • Provide referrals and linkage to appropriate supports and services
      • Coordinate and communicate to collaterals all aftercare planning
    • Provide coaching in support of decision making in both crisis and non-crisis situations.
    • Provide skills training for youth and parent/caregiver/guardian
    • Provide member transportation as needed in connection with service delivery.
    • Conduct member outreach as needed to engage members and families.
    • Participate in Risk management/safety planning.
    • Engage youth in social recreational athletic and artistic activities in the community.
    • Participate in all provider team meetings and collaborate and partner with youth family and collaterals to support the youths healthy functioning in the community.
    • Complete all required documentation in a timely manner.
    • Attend and maintain all trainings as assigned.
    • Attend and actively participates in supervision and staff meetings.
    • Ensure that all clients are treated with dignity and respect in accordance with Advocates Human Rights policy and a trauma informed agency.
    • Perform all duties in accordance with the agencys policies and procedures.
    • Strictly follow all agency Performance Standards.

    Additional Functions of the Senior Therapeutic Youth Support may include:

    • Lead group supervision or other groups.
    • Facilitate or co-facilitate staff trainings.
    • Assist with outreach to families waiting for services.
    • Assist with new staff training and orientation including mentorship training and ad hoc support.

Qualifications

  • Bachelors degree in social work counseling or related field required with one year minimum experience working with children/families and adolescents in child development childhood mental illness and family issues.
  • Must complete in CBHI training program upon acceptance of position and prior to providing services.
  • Must be able to perform each essential duty satisfactorily.
  • Ability to communicate effectively verbally and in writing.
  • Ability to develop and maintain satisfactory working relationships with employees and providers.
  • High energy level superior interpersonal skills and ability to function in a team atmosphere.
  • Commitment to Advocates values and mission.
  • Must hold a valid drivers license. Must have access to an operational and insured vehicle and be willing to use it to transport members.
  • Ability to read and speak English.
