Hospice (RN) Case Manager
A Hospice (RN) Case Manager is needed in Atlanta GA.

Salary and Benefits:
- Competitive Salary: Starting at $85000 - $92000
- Health Insurance: Employer covers 75%

Responsibilities:
- Monday Friday (8:30am 5:00pm)
- Serve as float nurse to provide coverage across clinical areas as needed in Roswell Atlanta and Covington
- Manage nursing care for assigned patients ensuring high-quality support and continuity of care
- Perform patient assessments and document findings accurately
- Assist in developing and supporting individualized patient care plans
- Communicate effectively with physicians and interdisciplinary team members
- Support patient and caregiver needs through education coordination and follow-up
- Visit 3 - 5 patients per day
- Collaborate with interdisciplinary healthcare team members
Requirements:
- Clear and active RN license in the state of Georgia
- Preferred experience in Hospice Home Health ICU CCU Oncology End-of-Life Care or MedSurg
- Compassionate and dedicated approach to patient care
