Job Description:
We are seeking a dedicated Physical Therapist to provide home health care this role you will deliver personalized physical therapy to patients in their homes helping them improve mobility strength and overall function.

Responsibilities:

Conduct comprehensive assessments to identify patient-specific physical therapy needs
Develop and implement individualized treatment plans
Deliver therapeutic exercises and interventions to achieve patient goals
Educate patients and families on home exercise programs and proper techniques
Collaborate with healthcare professionals to ensure coordinated care
Monitor and document patient progress adjusting treatment plans as needed
Maintain a safe and effective home therapy environment

Qualifications:

Licensed Physical Therapist in the state of Pennsylvania
Experience in home health or related healthcare settings preferred
Strong interpersonal and communication skills
Ability to work independently and manage a flexible schedule
Commitment to high-quality patient-centered care
