Job Description:
We are seeking a skilled Physical Therapist to join our home health team. This role focuses on delivering personalized therapy services in patients homes to improve mobility strength and overall function.

Responsibilities:

Assess patients conditions and medical histories
Develop tailored rehabilitation treatment plans
Conduct therapeutic exercises to improve mobility and physical function
Educate patients on injury prevention recovery techniques and home exercises
Monitor progress and adjust treatment plans as needed
Collaborate with healthcare professionals for coordinated care
Maintain detailed and compliant patient records

Qualifications:

Masters degree in Physical Therapy
Licensed Physical Therapist in the state of Pennsylvania
At least 1 year of home health or related experience
Current CPR certification
Valid drivers license and ability to travel to patient locations
Strong communication and interpersonal skills
Compassionate and patient-focused approach
