Job Title: CCS Service Facilitator

Overview:
Lutheran Social Services (LSS) is hiring a Service Facilitator for our Comprehensive Community Services (CCS) program in Columbia County. This role supports individuals with mental health needs through community-based care coordination across home school and other environments.
Work Setting:
- Primarily remote for documentation and meetings
- Telehealth services may be provided
- In-person client visits required throughout Columbia County
- Optional office space available in Baraboo
Compensation (Not Based on Billable Hours):
- Bachelors Degree: $24.20/hr
- Masters Degree: $27.20/hr
- Masters in training license: $28.20/hr
- Masters licensed: $30.20/hr
Key Responsibilities:
- Conduct assessments using standardized tools
- Develop and implement individualized service plans
- Coordinate and authorize services
- Facilitate team meetings and collaborate with families and providers
- Maintain accurate documentation and client records
- Participate in supervision training and staff development
- Flexible scheduling based on client needs (evenings/weekends may be required)
Qualifications:
- Bachelors degree in a human services field (Masters preferred)
- Valid drivers license and reliable transportation
- Strong communication organization and documentation skills
- Ability to work independently and collaboratively
- Proficiency with electronic health records and technology
