Job Description:The therapist is responsible for the evaluation diagnosis treatment and recommendation of services for enrollees. The therapist promotes optimal function and engagement in meaningful activities; provides support to families and collaborates with other disciplines as needed in order to address goals and objectives as indicated in each enrollees IFSP or compliance with state and local requirements each therapist will be responsible for maintaining appropriate documentation regarding enrollee services and subsequent needs.ESSENTIAL FUNCTIONS Evaluates diagnoses and manages enrollee impairments and disabilities. Provides therapy services to enrollees and initiates referrals when appropriate and in accordance with agency procedures. Conducts screenings of enrollees at regular intervals to determine need for interventions/treatment. Evaluates enrollees to obtain data necessary for treatment planning and implementation. Conducts specialized evaluations as indicated. Obtains appropriate physicians orders/documentation for treatment. Interprets and communicates evaluation findings to enrollees families and other team members. Develops coordinates and implements treatment plans with enrollees family and other team embers by identifying a target list including short- and long-term goals and methods to achieve identified goals. Assesses the need and makes recommendations for assistive devices orthotics and positioning and follows up as needed. Monitors enrollees response to intervention and modifies treatment as indicated to attain goals in accordance with plan and physician orders. Develops appropriate home therapies or programming within the community to maintain and enhance the performance of the enrollee in their environment. Documents all results of enrollees assessment treatment follow-up and termination of services. Provides in-service education to the members of the enrollees care team.
