Become the Driving Force Behind South Deltas Primary Care Network Transformation

Exciting opportunity for an experienced visionary health care leader to serve as the Primary Care Network (PCN) Director/Manager in South Delta. Reporting to the Executive Director you will play a pivotal role in operationalizing the Ministry of Healths PCN initiative within the region working in close partnership with the local Division of Family Practice health authority First Nations communities and other key stakeholders.

As the PCN Director/Manager you will oversee the day-to-day implementation of the PCN Service Plan leveraging your strong project management communication and facilitation skills to foster collaborative relationships and drive meaningful change. With a deep understanding of the PCN model and the principles of primary health care population health and health promotion you will be the driving force behind the transformation of primary care delivery in South Delta.

To succeed in this dynamic role you will need a minimum of 5 years experience in a distributed leadership capacity within the health sector as well as expertise in integrated service delivery design quality improvement and cultural safety. A post-secondary degree in a health-related discipline is required with a masters degree preferred.
