HCHB charting for visits
Physical therapists (PTs) evaluate diagnose and treat patients with physical impairments injuries or disabilities. Using evidence-based practices to improve mobility reduce pain and promote physical well-being.
Responsibilities:
- Conduct comprehensive physical examinations
- Assess patients movement patterns strength and range of motion
- Diagnose physical impairments and develop individualized treatment plans
- Design and administer therapeutic exercises manual therapy techniques and modalities (e.g. ultrasound electrical stimulation)
- Educate patients on self-care strategies home exercises and injury prevention
- Collaborate with other healthcare professionals such as physicians nurses and occupational therapists
- Monitor patient progress adjust treatment plans as needed and discharge patients when appropriate
- Provide patient education on health conditions treatment options and lifestyle modifications
- Masters or Doctoral degree in Physical Therapy from an accredited program
- National Physical Therapy Examination (NPTE) license
- State-specific licensure requirements
