Schedule: Monday-Friday 8 AM-5 PM (limited weekend coverage)

About the Role

We are seeking a dedicated and compassionate Advanced Practice Provider (APP) Family Nurse Practitioner (FNP) Adult-Gerontology Nurse Practitioner (AGNP) or Physician Assistant (PA) to join a leading post-acute care team. This position plays a key role in providing comprehensive medical care and continuity for patients transitioning through skilled nursing facilities (SNFs) rehabilitation hospitals and long-term acute care hospitals (LTACs).

Key Responsibilities

  • Conduct thorough patient assessments and review medical histories.

  • Order interpret and follow up on diagnostic tests (labs imaging etc.).

  • Diagnose and manage acute chronic and complex medical conditions.

  • Develop and implement patient-centered treatment plans including prescribing medications.

  • Collaborate closely with attending physicians rehabilitation specialists and nursing teams.

  • Educate patients and families on treatment plans medications and preventive care.

  • Maintain accurate and timely documentation within the EMR system.

  • Promote health wellness and disease prevention in post-acute environments.

Qualifications

  • Licensed Nurse Practitioner (FNP or AGNP) or Physician Assistant (PA) active and unrestricted.

  • Experience in post-acute care rehabilitation or hospitalist settings preferred.

  • Skilled in managing complex multi-morbid patients.

  • Strong understanding of healthcare regulations documentation standards and interdisciplinary collaboration.

  • Excellent communication and patient engagement skills.

  • Adaptable empathetic and team-oriented approach to patient care.
