About the Role
We are seeking a dedicated and compassionate Advanced Practice Provider (APP) Family Nurse Practitioner (FNP) Adult-Gerontology Nurse Practitioner (AGNP) or Physician Assistant (PA) to join a leading post-acute care team. This position plays a key role in providing comprehensive medical care and continuity for patients transitioning through skilled nursing facilities (SNFs) rehabilitation hospitals and long-term acute care hospitals (LTACs).
Key Responsibilities

Conduct thorough patient assessments and review medical histories.

Order interpret and follow up on diagnostic tests (labs imaging etc.).

Diagnose and manage acute chronic and complex medical conditions.

Develop and implement patient-centered treatment plans including prescribing medications.

Collaborate closely with attending physicians rehabilitation specialists and nursing teams.

Educate patients and families on treatment plans medications and preventive care.

Maintain accurate and timely documentation within the EMR system.

Promote health wellness and disease prevention in post-acute environments.
Qualifications

Licensed Nurse Practitioner (FNP or AGNP) or Physician Assistant (PA) active and unrestricted.

Experience in post-acute care rehabilitation or hospitalist settings preferred.

Skilled in managing complex multi-morbid patients.

Strong understanding of healthcare regulations documentation standards and interdisciplinary collaboration.

Excellent communication and patient engagement skills.

Adaptable empathetic and team-oriented approach to patient care.
