Job Summary

Referral Intake Specialist
The Referral Intake Specialist is responsible for coordinating and managing all incoming referrals intakes and placements for individuals seeking services through LEL Home Services and LEL Home Health. This position ensures that every referral is processed efficiently accurately and in accordance with all regulatory funding and agency requirements. The Referral Intake Specialist serves as a liaison among individuals families case managers discharge planners and community providersensuring a seamless transition into and out of services. This role embodies LELs mission and values by promoting person-centered care community inclusion and high-quality service coordination.
Reports to:Director of Referrals
Essential Duties and Responsibilities
  • Collaborate with individuals families case managers and external service providers to ensure seamless intake placement and discharge processes across both LEL Home Services and LEL Home Health.
  • Partner with the State Director of Program Services to maintain compliance with CARF standards Medicaid Waiver and all relevant state and federal regulations.
  • Communicate promptly with leadership regarding referral trends placement challenges and other key operational updates.
  • Take a proactive approach in identifying and resolving programmatic or service issues to ensure quality and continuity of care.
  • Champion person-centered best practices and LifeCourse principles supporting individuals in achieving their personal goals and meaningful community participation.
  • Apply Social Role Valorization (SRV) principles to enhance individual dignity self-determination and valued community roles.
  • Participate in continuous quality improvement (CQI) initiatives that positively impact service delivery family engagement and staff collaboration.
  • Demonstrate and model competencies of the Direct Support Professional (DSP) promoting excellence in all service delivery areas.
  • Work in collaboration with Human Resources to communicate with DSP and HHA applicants facilitating onboarding orientation and service readiness.
  • Maintain accurate and organized referral documentation intake records and communication logs.
  • Ensure timely coordination between LEL Home Services and LEL Home Health to support cross-program transitions and consistent service delivery.
  • Perform other duties as assigned by LEL Home Services or LEL Home Health leadership.

  • Support quality assurance and continuous improvement initiatives within both divisions.
  • Participate in interdepartmental meetings to align referrals staffing and service capacity.
  • Report key referral trends barriers and opportunities for service growth to leadership.
Qualifications:
  • Associate degree in Healthcare Administration Legal Studies Human Services or related field preferred.
  • Bachelors degree in social work Psychology or Healthcare or equivalent experience supporting individuals with preferred disabilities.
  • Knowledge of Medicaid Medicare waiver programs and CARF accreditation standards.
  • Strong organizational analytical and problem-solving skills.
  • Excellent interpersonal written and verbal communication abilities.
  • Proven ability to manage multiple priorities while maintaining accuracy and attention to detail.
  • Customer serviceoriented with a professional and compassionate demeanor.
  • Proficient in Microsoft Office and electronic records management systems.
Physical Requirements:
  • This position operates in a remote/home office environment and routinely uses standard office equipment such as computers phones and video conferencing platforms.
  • The employee must be able to sit for extended periods and work effectively using a computer and other digital tools.
  • The employee must be able to communicate clearly and effectively through verbal and written channels.
  • Occasional travel may be required for meetings training or agency events.
  • Must have a reliable internet connection and designated workspace that supports confidentiality and productivity.
While performing the duties of this job the employee is regularly exposed to an office environment in which the noise level is usually low to moderate. The employee is occasionally exposed to outside weather conditions as well as Client residences in which the noise levels are usually moderate.
