ESSENTIAL DUTIES FUNCTIONS AND RESPONSIBILITIES
- Assists in providing and facilitating therapeutic activities such as coordination of individual group and family session(s)/psychoeducational group(s) for outpatient mental health clinic patients.
- Provide assistance in case management and care coordination to outpatient mental health clinic patients with acute and chronic mental health problems including depression suicide ideation bereavement psychotic features family conflicts substance abuse or dependence and various traumatic experiences.
- Coordinates outpatient mental health navigation services in consultation with psychiatric medical providers therapist case workers and other professional colleagues and agencies to promote optimal patient care such as referrals for psychiatric evaluation and treatment including medication management.
- Participates and when necessary coordinate individual group family and psychoeducational psychotherapy sessions with patients. This is inclusive of areas such as but not limited to: parenting relapse prevention early recovery skills and grief/loss support group.
- Assists in the development and coordination of group therapy and support group activities for outpatient level of care.
- Works closely with other members of the interdisciplinary treatment team during the formulation of patients plan of care.
- Provides direct supervision and behavioral health interventions as needed to patients in the outpatient setting. This may include patients requiring transfer to higher level of care and needing periodic follow-up.
- Maintains appropriate records by documenting observations and therapeutic interventions provided during patient care in the Electronic Health Care (EHR).
- Participates in and/or initiates educational preventative or advocacy efforts to meet the mental/behavioral health needs of patients and the community.
- Assists psychiatry medical and nursing staff with relevant duties as it relates to delivery of services with outpatient psychiatric follow-up and rendition of care for high/moderate risk patients needing optimal psychiatric follow-up. Works closely with Medical Assistant with identification of staffing and appointment management for psychiatric medical provider(s).
- Assist in keeping inventory of medical supplies for Outpatient Mental Health Injection clinic and ordering su8pplies when needed. Assist Medical Assistant with contacting pharmacy service when patients present for injection.
- Participates in consultation with FDIHB personnel health professionals and public and private resource agencies to assist patients in resolving medical social familial and behavioral health problems as needed.
- Performs other duties as assigned.
MANDATORY MINIMUM QUALIFICATIONS
Experience:
Two (2) years of direct work experience in a healthcare setting.
Education:
High School Diploma or Equivalency (HSE)
