Community of Hope is seeking a Perinatal Case Manager Specialist HONEY (Housing Our Newborns Empowering You) to be a trusted guide for mothers and families during one of lifes most important transitions. Youll ensure access to prenatal and postpartum care provide education on maternal and child health and connect patients with community and parenting supports. Through collaborative care planning youll help each patient move toward their personal health and family goals. This position is split between our Conway Health and Resource Center in SW DC and Virginia Williams Resource Center in NE DC.

What Youll Do
- Maintains a case load of about 30 participants who are referred from the perinatal navigators at the Virginia Williams Family Resource Center providing care coordination services to ensure access to perinatal health and early child health services
- Completes biweekly home and community visits to engage families in their natural environment assess needs monitor progress toward goals and ensure interventions are responsive to each familys home environment
- Provides health education to clients in a variety of settings with an emphasis of increasing understanding of healthy pregnancy self- management of care and health systems
- Develops care plans with designated periods with particular emphasis on risk factors including but not limited to social needs barriers to care pregnancy planning and food insecurity
- Assists as a liaison between the client and their families homeless services provider clinical staff and other health care professionals.
- Participates in Virginia Williams Resource Center staff huddles (currently every other Friday virtually or in person).
- Secures client information and maintains client confidence by completing and safeguarding medical records and keeping client information confidential.
Requirements
Must-Haves
- Experience providing social services care coordination or health care education required.
- Familiarity with perinatal services and standard of care required
- Strong organizational skills
- Strong written and verbal communication skills
- Ability to travel between sites required
- Ability to conduct home visits. Valid drivers license and vehicle required as well as proof of auto insurance required.
- Willingness and ability to make home visits and hospital visits and work occasional evenings and weekends required
- Proof of required vaccinations is required. This includes but may not be limited to Flu and Covid. COH will consider requests for reasonable accommodations for anyone who cannot be vaccinated for a religious or medical reason subject to applicable law
Nice-to-Haves
- B.A./B.S. or equivalent experience required.
- Ability to multi-task and problem-solve.
- Experience working with an under-resource population including pregnant people preferred.
- Familiarity with the Medical Assistance system and community resources strongly preferred
