Care Integration Sr Aide LHCSA

Position Summary: The Care Integration Senior Aide (CISA) will be deployed under the supervision of the Clinical Team to visit clients homes and work with home care workers on designated cases. The CISA will act as a liaison between the home care worker the client/families and the Interdisciplinary Team (IDT) with the goal of improving client outcomes.

Principal Responsibilities:

• Assist and conduct in-home visits as directed by Clinical Team with input from Coordinator Team
• Observe record report information regarding the clients health and social conditions and compliance with the care plan
• Support home care workers in developing trusted relationships and establishing open communication with clients and families using coaching techniques.
• Reports observations to the IDT. Participates in IDT meetings as a member of the team and completes follow up as directed.
• Retain all certification requirements of a Home Health Aide.
• Adhere to organizations standards by completing annual mandatory trainings in a timely manner on topics including but not limited to Sexual Harassment Prevention Training IT Security Awareness Compliance OSHA HIPAA and any other training the organization may deem necessary in regard to regulatory compliance or good business practices.

Salary Range: $23 - $25 per hour commensurate with experience.

Requirements:

• Educational: Certified Home Health aide. Demonstrated ability to perform Home Health Aide job duties.
• Experience: minimum one year as a Home Health Aide with Selfhelp and familiarity with Paraprofessional Policies and Procedures.
• Experience with HHA Exchange and the Mobile App
• Problem solving abilities while multi-tasking; ability to exercise sound judgment
• Excellent communication skills. (ability to speak read and write functional English.)

Working Condition/Physical Demand

• Business Office Environment with phone and computer use.
• While performing duties of this job the employee may occasionally be exposed to outdoor weather conditions moving mechanical parts and occasional risk of exposure to pathogens.
• The employee may frequently lift and/or move up to 25 pounds and occasionally lift and/or move up to 50 pounds.
