Key Responsibilities
Implement individualized care plans under the supervision of a licensed Physical Therapist
Conduct follow-up visits and provide therapeutic exercises and treatments
Educate patients and caregivers on wellness safety and the use of assistive devices
Maintain accurate documentation in compliance with legal and professional standards
Ensure adherence to infection-control and safety protocols
Travel to patients homes as part of daily caseload

Qualifications
Required
Associates degree in Physical Therapy
Active state license to practice as a Physical Therapist Assistant in Texas
Minimum 1 year of experience in home health or related physical therapy setting
Valid CPR certification
Valid drivers license and ability to travel between patient locations

Compensation and Benefits
Pay Rate: $35 $45 per hour (Full Time Direct Hire)
