Key Responsibilities

Collaborate with physicians nurses and other healthcare professionals to coordinate care.
Monitor and document patient outcomes adjusting treatment as needed.
Maintain compliance with clinical legal and regulatory standards.
Qualifications
Required
Valid Texas Physical Therapist license.
Current CPR certification.
Excellent communication organizational and time-management skills.
Ability to work independently and manage a home health caseload.
Preferred
One or more years of home health or outpatient experience.
Strong interpersonal skills and patient-centered approach.
Compensation and Benefits
Salary: $105000 $115000 annually based on experience.
