Job Summary:
The Registered Nurse will provide skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. Daily tasks include patient assessments medication administration treatment planning and collaborating with the interdisciplinary team. Strong communication and collaboration with patients providers and other team members is critical. The position may require floating between units within the facility.

Required Skills & Certifications:
- Active Registered Nurse license in the state of working

- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Bachelors degree in Nursing or related field
- Minimum 2 years of hospital home health or hospice care experience
Preferred Skills & Certifications:
- Not specified
Special Considerations:
- Level II Trauma Center
- Level II Pediatric Trauma Center
- May float between units
- Scrub Color: Any color for RN
- 48-hour shifts may be approved
Scheduling:
- Weekends every other
- No on-call requirement
- No block schedules required
- Expected ratios: 1:2
