- Conduct patient assessments to determine care needs.
- Administer medications and treatments as prescribed.
- Develop and implement patient care plans.
- Collaborate with physicians and other healthcare professionals.
- Provide skilled nursing care in various settings (hospital home hospice).
- Document patient information accurately and timely.
- Communicate effectively with patients families and the healthcare team.
