Job Summary

Overview

This part-time position supports an intensive case management and linkage program aimed at serving veterans and military families. The program focuses on preventing risks such as suicide homelessness and underemployment among veterans while promoting protective factors including social connectivity financial stability resilience and socio-emotional skills development.

The program is transitioning to a Full-Service Partnership (FSP) model providing comprehensive services including mental health care housing support medication management and 24-hour crisis intervention. Referrals are triaged by risk level with low-risk individuals connected to community services and moderate to high-risk individuals receiving care through the FSP. This hybrid approach aims to improve access and engagement by offering a range of services beyond traditional mental health care.

The program also collaborates with federal and local housing partners to provide permanent housing solutions for veterans experiencing homelessness.

Position Details

Part-time psychiatry relief position
Two 10-hour days or three 8-hour days per week
On-site work required including daily field rounds with clients
Documentation and prescription responsibilities following field rounds
Possible flexibility to finish workday at an alternate location after field rounds
No telehealth options currently available
