Performs case management/service coordination activities that enable Cook Childrens Health Plan members with chronic conditions or complex needs to achieve an optimal level of health and independence through advocacy assessment planning communication education resource management care coordination collaboration and service facilitation. This is an office-based position with required field work. Driving and in home visits are required up to 75% of the time. Some weekends may be required to meet member needs.Additional Information:
This Social Worker will do in-home visits with children birth to 21 years of age and their families covering Wise Denton Parker Tarrant Johnson and Hood counties.Duties include case management/service coordination activities that enable Cook Childrens Health Plan members with chronic conditions or complex needs to achieve an optimal level of health and independence through advocacy assessment planning communication education resource management care coordination collaboration and service facilitation. This is a home office-based position with required field work. Driving and in home visits are required up to 75% of the time. Some weekends may be required to meet member needs. Reimbursed for travel.
Required Education/Experience and Licensure:
Master of Social Work (MSW) degree.
Licensed Master Social Worker (LMSW) or (LCSW) in Texas.
Two (2) years of experience working with vulnerable populations and people with chronic or complex conditions.
Strong skills in medical record review and/or behavioral health assessment is vital; Prior MCO experience a plus.
Must possess the ability to function in an interdisciplinary team environment.
Self-starter with the ability to be effective in a large complex and highly matrixed organization is a must.
Need to possess the ability to focus on both the details and the big picture simultaneously.
Strong relationship-building written and verbal communication skills.
Critical thinking and problem-solving with the ability to plan and organize effectively.
Bi-lingual (English/Spanish) Preferred.
Proficient in typing and in use of computer applications (Excel and Outlook).
