BAYADA Home Health Care is seeking aRN Case Manager for our Wyomissing PA Pediatrics and Adults home care office. This office provides best in class care to clients throughout Wyomissing PA and surrounding areas.

Responsibilities for a RN Case Manager:
- Lead inspire and motivate others to provide exceptional care.
- Visit our Pediatric/Adult clients and conduct assessments in Wyomissing PA and other surrounding areas.
- Develop the care plan and oversee its execution.
- Supervise LPNs RNs and home health aides (HHA) staff in the field.
- Facilitate new hire orientation.
- Conduct competencies on new and tenured staff.
Qualifications for a RN Case Manager:
- Registered Nurse (RN) with 1 or more years of experience as an RN.
- Supervisory experience preferred.
